"Navn" means "Name". Pretty obvious?
"Epost" means "Email". That wasn't too hard? The little clicky box is for showing your email public or not. The text "epostadresse vil ikke bli utnyttet til spam" means that your email won't be used for spam.
"(og adressen blir manipulert slik at de ikke blir fisket av spamroboter)" means that your adress will be manipulated, so spambots can't fish'em up.
"Bosted" means "place you live" or something. Just type in your country, I think.
"Yrke" is "what you work with" or something like that (my english vocabulary isn't excactly complete). Write "student" or whatever you do for a living.
"Kommentar" means "comment". Say exactly what you want, to the record company EMI, about copy protection.
This was basically everything you need. But, to make this a little more trustworthy, I can translate the text you are signing for.
